Registration Information and Forms

All students, including high school students, must be accompanied by a parent or guardian in order to register. Remember to have your student's social security number with you. Proof of insurance is required for all sports. Most forms needed for registration can be found at the bottom of this page.

Students entering Pre-K, Kindergarten, 6th and 9th grades must submit a school health physical with up-to-date immunization (shot) record at registration. Dental Exams are also required for Pre-K, Kindergarten, 2nd, and 6th grade students.

Registration Forms and Other Information

Below are forms and information needed at the time of registration. The left column contains fillable forms which means you can type your information directly into the form before printing it. The right column contains forms and informative documents that are non-fillable which means you must print them and fill them out by hand. Please note that due to time constraints not all forms may be found on this page, other forms may be required to be filled out at the time of registration.

When using the fillable pdf forms, you should have an option at the top right of the page to "Highlight Fields", click this option to see what fields are fillable. Please note that all signature fields are non-fillable. After completing a form, use the print button on the form itself to print the document and then use the clear button to clear the information.

For the "Student Handbook/Acceptable Use Policy" form (for all students), this document states that you have read and understand the rules and information located in the Student Handbook, this includes our Acceptable Use Policy. A Student Handbook will be given to you at registration and is also located on our site. Please print, fill out, and sign the form.

For the "Free and Reduced Lunch" form, please only read, print and fill out pages 3 and 4, this is all that is needed by the district.

Eye Examination Information

An Eye Examination Report form or Eye Examination Waiver form is required for all new students. Please read the following information from the Illinois State Board of Education to help you decide which form you need and then print and fill out the form.
